To get Webchat working following should be included in the <head> tag

In order to auto start web chat window on page load, following javascript needs to be added inside the <head></head> tags. The other option is to add an onclick event to an HTML object.

In order to create custom form fields following format should be used:

To disable comments field box a third parameter needs to be set to false:

To add a graphic to the intiating chat dialog add graphic to the "Content" folder and add the name to the 4 paramater. Like wise for the chat window:
